Reading Entrance Criteria: Grade 1-5 students with and NWEA score in the 40%ile or lower, reading below grade level with an intensive need, and/or an MSTEP score of Not Proficient. Data points will be from the Benchmark Assessment System, NWEA, MSTEP. Kindergarten students failing or at risk of failing to meet grade level expectations as determined by classroom teacher formative and summative assessments, beginning in November, NWEA score in the 40%ile or lower when this data is available.
Person responsible for intervention: Title One staff and classroom teacher.
Responsibilities: Record baseline reading level. Targeted interventions will be used for specific areas such as phonological awareness, fluency, phonics, vocabulary and comprehension. Informal progress monitoring will be ongoing. Benchmark testing will take place via the classroom teacher every 6-12 weeks, depending on the grade level. NWEA testing occurs three times each year. The MSTEP is given each Spring to students in grades 3-5.
Reading Exit Criteria: Student is reading within one level of expected grade level, an NWEA score above the 40%ile and/or has met projected NWEA growth.
Math Entrance Criteria: Grade 1-2 students with >50% of report card outcomes marked as a 1, and/or an NWEA score in the 40th%ile or lower. Grade 3-5 students with >50% of report card outcomes marked as a 1, an NWEA score in the 40th%ile or lower, and/or an MSTEP score of Not Proficient. Kindergarten students failing or at risk of failing to meet grade level expectations as determined by classroom teacher formative and summative assessments, beginning in November. An NWEA score in the 40%ile or lower when this data is available.

Responsibilities: Collect baseline data, provide student support (individual or small group), maintain student support records, ongoing student assessment.
Math Exit Criteria: Met NWEA growth projection, and/or an NWEA score above the 40th%ile.
